For your child to be accepted into a Representative Group in the Music Festival, all parts of this confirmation must be completed 7 July.
Representative Group – Acceptance of Place
There are THREE parts to this confirmation; please ensure that you complete ALL parts below that apply to your child, and make payment of the $60 fee by internet banking.
If, for any reason, you are unable to accept the place, complete the Decline Place information only in the form.
Part 1: Accept or Decline
Please check each item to indicate that BOTH you as parent(s) AND your child agree to each commitment. When you have completed the list, click the “Submit return” button.
Note: If you are unable to agree to all items, we cannot confirm your child’s place in the Music Festival.

Part 2: Fee payment & tickets
Fee payment
Payment of the registration fee confirms acceptance of the place in the Representative Group.
Internet Banking:
Please make payment online to: CSMFA 02 0820 0023240 00
When paying the $60 online, you MUST enter the acceptance number (on the separate email you received) in the reference field and the child’s name in the particulars box. If this is not done then we will not be able to match the payment.
If payment of this fee presents a barrier to the taking up of the place, contact the Festival Business Manager. Assistance may be available.
Tickets
These can be purchased direct from Ticketek (www.ticketek.co.nz).
Pre-sale Representative family tickets will open on Thursday 22 August (Public sale opens on 2 September). Ahead of that date we will supply Rep group members with the promo code for pre-public sales tickets.
Part 3: Four-Year Medal (Year 8 pupils only)
Four Year Medals are awarded to students who have been a member of a Representative Group for four years consecutively, each year from Year 5 up to and including Year 8. To receive a Four-Year Medal, you MUST complete the online form.
